Required Forms (All Campers)

Please complete the steps below for each child:

1. Complete the online forms once for EACH CHILD.

2. Confirm your email address by clicking “Confirm my email address” in the Adobe Sign email after submission.

3. Submit all forms by May 1.
   Friend requests will not be accepted after May 1.

Immunization Records

Please submit a copy of your child’s immunization records using one of the following methods:

Medications

If your child will bring medication to camp, please provide a doctor’s note.
(A note typically used for school is acceptable.)
